If you are unable to complete our online adoption application (preferred method), please thoroughly read and fill out the application below. The information you provide in this Application and during our interview will assist us in finding the best companion animal match for your needs.
Applications are reviewed and processed based on the order of submission in conjunction with what is considered the best match based on what we know of the animal’s needs. We make every effort to respond to each application within 48 hours. Your patience is much appreciated! Please note that as our office is closed on weekends, applications may not be reviewed until the following business day.
Once you have clicked the “Submit Adoption Application” button below, you will receive a confirmation notification. If you do not receive a confirmation notification, your application has not been submitted. Please try submitting again. Thank you!
CCHS requires the following adoption fees:
- Dog (>6 months): $175.00
- Puppy (<6 months): $225.00
- Cat (>6 months): $90.00 or $150.00 for two
- Kitten (<6 months): $130.00 or $200.00 for two
